I grant NPT HealthWorks the non-exclusive, royalty-free, worldwide, perpetual, and irrevocable right to use, reproduce, modify, adapt, publish, translate, distribute, and display the content in whole or in part, and to incorporate it into other works, in any form, media, or technology now known or later developed, for the purposes of promotion, marketing, advertising, and public relations.

I understand that once the content is posted on social media platforms, it may be shared, re-shared, and disseminated by other users, and NPT HealthWorks cannot control the further distribution of the content.

I acknowledge that I will not receive any compensation, financial or otherwise, for the use of the content as described herein.

I represent that I am the rightful owner of the content or have obtained all necessary permissions, licenses, consents, and releases from third parties (including but not limited to any individuals appearing in the content) to grant the rights and licenses set forth in this consent form.

I waive any right to inspect or approve the finished product, including any written copy or edited versions, wherein my name, image, likeness, or content appears.

I release, discharge, and hold harmless NPT HealthWorks, its employees, agents, and representatives from any and all claims, demands, actions, suits, or proceedings of any kind arising out of or related to the use of the content as described herein, including but not limited to claims for defamation, invasion of privacy, or infringement of rights of publicity or copyright.
